FAMILY APPRECIATION PROGRAM AWARD
Purpose of Award: To recognize families of Volunteer Fire/EMS members whose families must sacrifice time with the member for the good of the community.
Award Sponsor: Maryland State Firemen’s Association Recruitment and Retention Committee
Award Administered By: Volunteer Recruitment and Retention Committee
Award Rules:

Open to any volunteer member of the Maryland fire/EMS service who has a family (spouse, child, children.)

Member MUST be active in their capacity as a member.

Candidates will be nominated by their company, with the form signed by the chief, president, or other officer using official nomination form.

The committee will divide the State into various regions using the MFRI regions as models.

A sub-committee of the MSFA – Volunteer Recruitment and Retention Committee will select the winners based on the nomination form content and the following factors:
a. Number of entries versus numbers of packages available
b. Evaluation of nominations by regions
c. Each region will have a proportionate number of awards

A lottery may be used to determine the final winners if more qualified applicants exist than awards available.

Nominations must be received by May 1st.

Awards will be presented at the MSFA Annual Convention and Conference

Awards will be redeemed in accordance with the vendors conditions.

Members of the Volunteer Recruitment and Retention Committee, Executive Committee and MSFA Presidents are excluded from the program.
Family Appreciation Program Award Nomination Form
Nominee must:
· Exhibit consistent positive effort toward training, response, and administrative duties
· Exhibit consistent positive effort to attend meetings, help with fund raising, and work on committees
· Exhibit overall effort to support the team concept (NOTE: Not necessarily the team leader, but an active member of the team)
· Active member for at least one year
· Narrative of 250 words or less …. Typed or printed. This award is NOT based on a single act of heroism, but rather the consistent community volunteer effort in the fire/rescue service
